fix: sync .env.example with actual config

Browse Source 
Remove vars not read by code (DB_MAX/MIN_CONNECTIONS, STORAGE_BACKEND,
PRODUCTION, JWT_EXPIRY_HOURS). Add missing NATS_URL, RUST_LOG.
Fix PORT default 3000->8000, CORS origin to match.
