refactor: remaining MEDIUM — CQRS splits, DI Deps, profile dedup, event Value, response enum

M1: MovieRepository→MovieCommand/MovieQuery, WatchEventRepository→

M2: goals/ and import/ use Deps structs
M7: extract upload_image helper in update_profile
